Kindergarten registration for the 2012 – 2013 school year will be held in Derby the week of March 12th through March 16th at both Bradley and Irving Schools from 9:00 a.m. to 11:00 a.m. Monday through Friday.
Parents may register any child who will be five years old on or before January 1, 2013. In order to register parents must have the child’s birth certificate and must show proof of residency in the school district.
Parents must also show proof of two measles, mumps and rubella vaccines(MMR), at least three of the polio series, no less than four of the DPT series and at least one HIB for children not five when school starts.
The most recent DPT and polio vaccines must be given on or after the child’s fourth (4) birthday.
Children must also have the series of 3 Hep B shots and the chicken pox vaccine or medical proof of having had the disease.
A physical examination by a physician during the current year is also required before entrance to kindergarten in September.
A state physical health form will be given to parents upon registration. This form is to be completed and signed by both the parent and the physician.
